The law belongs to everyone.
So the forms should, too.
A free, comprehensive library of Louisiana legal forms — built for the people who can't afford a lawyer but still deserve their day in court. Divorce, custody, successions, expungement, eviction, protective orders, and more — standardized, statute-cited, and ready to file in any Louisiana parish.
Bridging the gap to justice, one form at a time.
Going to court without a lawyer is one of the hardest things a person can do — and in Louisiana, it happens every day, in every parish. Rent is due. A custody hearing is looming. A loved one has passed. A protective order is needed tonight. The paperwork stands between a person and their rights, and that paperwork is written in a language designed to exclude the people who need it most.
This library exists to change that. Every form here has been collected from official Louisiana sources — the Supreme Court, the Access to Justice Commission, parish clerks, and the Louisiana State Bar — then standardized, cleaned, and organized by subject matter. Every form cites the Louisiana statutes that govern it. Every section includes plain-language instructions about what the form does, when to use it, and what traps to avoid.
We don't replace lawyers. We make sure that when you can't afford one, you're not starting from zero.
Justice should not turn on the depth of a person's pocket. The mission of this project
Twenty-four areas of Louisiana law — one library.
Every form cross-referenced to the Louisiana Civil Code, Code of Civil Procedure, Code of Criminal Procedure, and Revised Statutes. Download in Microsoft Word format and fill in the blanks.

If you have a question about a form, need legal advice, want to report a correction, or think a form should be added to the library, email labayoujustice@gmail.com or fill out the short form on the Contact page. Corrections from clerks of court, law librarians, and legal aid staff are especially welcome.

Downloading, reading, or using any form on this website does not create an attorney-client relationship. The forms are general templates and may not fit every situation. Louisiana law changes, and the form you need may require customization. For advice specific to your case, consult a Louisiana-licensed attorney. See the full Disclaimers for details.
