Disclaimers & Legal Notices.
The law requires certain disclosures on any website operated by an attorney or providing legal information. This page collects all of them in one place. Take a moment to read them.
1. Not Legal Advice
All content on this website — including forms, form instructions, articles, essays, directories, and any other material — is provided for informational purposes only. It does not constitute legal advice and must not be relied upon as a substitute for consultation with a Louisiana-licensed attorney.
Law changes. Statutes are amended, cases are decided, court rules are revised, and local practice varies from parish to parish and division to division. A form that is appropriate for one person's situation may be inappropriate or incomplete for another person's. The correct application of any law to any specific set of facts requires the professional judgment of an attorney who has evaluated the specifics of your case.
The forms in this library are templates drawn from publicly available Louisiana court forms, legal aid sources, the Louisiana Supreme Court, the Access to Justice Commission, the Louisiana State Bar Association, and the drafter's own work. They are offered as starting points, not finished products. You are responsible for verifying that any form you use is current, accurate, and appropriate for your situation before filing it with any court.
Do not rely on this website as your only resource. Contact one of the legal aid organizations, the LSBA Lawyer Referral Service (1-800-421-LSBA), or a private Louisiana-licensed attorney.
2. No Attorney-Client Relationship
Using this website, downloading forms, reading articles, sending email, or submitting the contact form does not create an attorney-client relationship between you and the operator of this site, any attorney associated with this site, or any law firm. An attorney-client relationship arises only when:
- You have spoken directly with an attorney who has confirmed there is no conflict of interest preventing representation;
- The attorney and you have mutually agreed on the scope of the representation; and
- You and the attorney have signed a written engagement letter memorializing the agreement.
Until all three of these things have happened, you are a prospective client and not a client. Communications between you and the attorney during the prospective-client phase are not protected by the attorney-client privilege in the same way as post-engagement communications.
Please do not send confidential information through this website or by email until an attorney-client relationship is formally established. The contact form asks for only the minimum information needed to run a conflict check; detailed facts about your case should be discussed only during a consultation after that check is complete.
3. Attorney Advertising Notice
This website is attorney advertising within the meaning of the Louisiana Rules of Professional Conduct, Rule 7.2. It is operated by a Louisiana-licensed attorney in good standing with the Louisiana State Bar Association. The attorney is responsible for the content of this website.
Attorney advertising is regulated in Louisiana and this website complies with the requirements of Rules 7.1 through 7.10. This site does not guarantee any particular outcome for any particular client. Prior results described on this site (if any) do not guarantee a similar outcome in any future matter.
The choice of a lawyer is an important decision and should not be based solely on advertisements or self-proclaimed expertise. A person considering retaining any Louisiana attorney is encouraged to review the attorney's credentials, experience, and disciplinary record (if any) through the Louisiana State Bar Association at lsba.org.
Responsible Attorney: [Attorney name] — [Louisiana Bar Number] — [City of primary practice], Louisiana.
This information will be populated when the site goes live.
4. Privacy
This website is designed to collect as little personal information as possible. Specifically:
- No tracking cookies. This site does not use analytics cookies, advertising cookies, or third-party tracking.
- No data collection from browsing. Simply visiting pages, reading articles, or downloading forms does not log your identity.
- Contact form submissions are transmitted via email (using your own email client) and are stored in the recipient's email system under ordinary professional confidentiality practices. The submission itself is not stored on this website's servers.
- No sale or sharing of personal information. Any information you voluntarily submit is used only to respond to your inquiry and, if you become a client, to represent you. It is never sold, traded, or shared with third parties for marketing purposes.
- External links. When you click a link to another website, you are subject to that site's privacy policy, not this one.
If you have questions about privacy practices or want your information removed from any records (subject to any legal obligations to retain it), please use the contact form.
5. Scope, Accuracy, and Jurisdiction
This website addresses Louisiana law only. The forms are designed for use in Louisiana courts, applying Louisiana statutes, Louisiana Code of Civil Procedure, Louisiana Code of Criminal Procedure, the Louisiana Civil Code, and Louisiana District Court Rules. They are not appropriate for use in other states or, except as noted for specific forms, for federal court matters.
Louisiana is a civil-law jurisdiction and its legal system differs significantly from the common-law systems of the other 49 states. Forms and procedures that work elsewhere will often not work in Louisiana, and vice versa.
This website makes every effort to ensure accuracy, but no warranty, express or implied, is given as to the correctness, currency, or completeness of any content. Forms may contain errors. Statutory citations may be outdated. Court information may change between the time this site was updated and the time you visit. Before relying on any content, verify it against primary sources:
- Louisiana statutes and the Civil Code: legis.la.gov
- Louisiana Supreme Court rules and forms: lasc.org
- Local court rules: the website of the specific court where you will file
- Louisiana State Bar Association: lsba.org
6. External Links
This website links to external resources maintained by third parties — courts, bar associations, legal aid organizations, government agencies, and others. These links are provided for convenience and reference.
This website does not endorse, sponsor, or guarantee the content, accuracy, or continued availability of any linked external resource. Information on an external site is that site's responsibility, not ours. Privacy practices on linked sites are those sites' responsibility. If an external link becomes broken or redirects somewhere unexpected, please use the contact form to let us know.
7. Copyright, Attribution & Permitted Use
The forms in this library are compiled from public sources including the Louisiana Supreme Court, the Access to Justice Commission, the Louisiana State Bar Association, parish clerks of court, and various judicial districts, many of which publish their forms for free public use. Some forms have been modified, standardized, or newly drafted for this library.
You are free to:
- Download any form on this site and use it for your own legal matter at no cost;
- Share links to this site with friends, family, clients, or community members;
- Print and distribute forms to individuals who need them for their own cases;
- Reference this site in academic, journalistic, or advocacy work with attribution.
You may not:
- Republish this site's content under a paywall or for commercial resale;
- Remove attribution from forms and present them as your own product;
- Use the site's name, branding, or identity in a way that suggests affiliation you don't have.
Mandatory forms issued by the Louisiana Supreme Court — including Louisiana Protective Order Registry forms (under La. R.S. 46:2136.2(C)) and the uniform expungement forms — are the property of the Court and may not be altered. They are reproduced here unchanged and retain their mandatory character.
If you have questions about permitted use, reuse, or collaboration, please get in touch.
8. Changes & Updates
This website, including these disclaimers, the forms library, and the court directory, is updated periodically. The version of any page in effect is the version posted at the time you access it. Material changes to policies affecting the site's legal scope will be noted on this disclaimers page with a revision date.
Last updated: Initial publication.
